Young Persons Membership Form

We are very pleased to welcome you to Cheam Cricket Club this season.

To ensure we have the correct contact details for you, please fill out this form electronically below or complete a Membership Form and post to the address advised. If you are under the age of 18 please also ask your parents / carer or legal guardian to sign the Membership Form before it is returned or acknowledge on this electronic form.

We will also use this information to ensure that you are kept informed about events and information from Cheam Cricket Club.

By submitting the form:  

• I agree to my child in my care taking part in the activities of Cheam Cricket Club
• I understand that I will be kept informed of activities at Cheam Cricket Club – for example times and transport details etc.
• I understand in the event of injury or illness all reasonable steps will be taken to contact me / the alliterative contact and to deal with that injury/illness appropriately.
• I acknowledge and will abide by the Cheam Cricket Club code of conduct for members and guests and the code of conduct for young people.

Once the form has been submitted please make payment by either cheque (payable to Cheam CC) or electronically to the back details displayed on the following screen. Please use your child's name as a reference. To assist us please submit one payment electronically for each child.
